I/ we understand the dangers of breathing radon, a radioactive gas and a class A carcinogen and the second leading cause of lung cancer, estimated to cause 21,000 deaths in the United States annually, and that radon can only be detected with a radon test. Select either Decline or Accept Radon Test Below: Please Initial Here *__________Decline Radon Test: Against the recommendation of the EPA, the Surgeon General , and/or my loan originator, and/or my real estate agent, and/or my home inspector, I/we choose not to have a home radon gas test conducted. I/we therefore assume all liability for elevated radon concentrations the test would have revealed, if discovered after the closing of the transaction. *__________Accept Radon Test: I/we choose to have a home radon test performed according to EPA Protocol. I/we understand that there is an additional fee of $100.00 for this test. Waiver of Mold Testing and Release Please Initial Here: *_________Mold Testing: I/we the undersigned have been informed and understand the following: I/we have been advised by Homeguide Home Inspection that the subject property may be subject to contamination by mold. Molds produce tiny spores that reproduce. Mold spores waft through the indoor and outdoor air continually. When mold spores land on a damp spot indoors, they may begin growing and digesting whatever they are growing on in order to survive. There are molds that grow on wood, paper, carpet, and foods. When moisture or water accumulates indoors, mold growth will often occur, particularly if the moisture problem remains undiscovered or un-addressed. I/we have been advised of the potential health effects and symptoms associated with mold exposures including allergic reactions, asthma, and other respiratory complaints. There is no way to determine if there is mold and if it is a health concern without testing. I/we hold Homeguide Home Inspection, it’s agents and employees, harmless and free from all liability and legal action relating to any presence of Mold at the subject property, regardless of the legal theory upon which any such claim rests. This waiver and release shall be binding on all my heirs, agents, assignees, successors, and on any other person(s) who might otherwise be entitled to file suit or make a claim on my behalf. *______________Accept Mold Testing: I/we choose to have mold Testing performed according to EPA Protocol. I/we understand that there is an additional fee for this testing. A mold swab test will be taken at each different location where Evidence of Microbial organisms are observed. An indoor and an outdoor air quality test will be performed where there is a suspicion of any microbial organisms or a musty odor. A carpet scrapping can also be taken when there is a suspicion of past microbial organism, to create a history.Fees: $150.00 per swab -Up to 1 or 2 swabs. $100.00 per swab after 3rd swab, or more $150.00 per air quality test – 1 indoor - 1 outdoor (minimum of 2 required) $125.00 per carpet swipe test.
